Capstone Topic Summary, in pressure ulcer prevention in a hospice setting
Pressure ulcer or a bed sore is the tissue damage of the localized area that can even be extreme as a necrotic tissue. It can be caused by many reasons such as immobility, lack of sensory perception, poor hydration, poor nutrition, incontinence or any disease condition that can impact blood flow or the cardiovascular system.
Prevention in a hospital setting includes the following points:
*Good hygiene
*Support surfaces inorder to prevent pressure faced by the body
(Static supports or dynamic supports can be used)
*prevent pressure, friction and shear as possible
(Padding with transparent, hydrochloride or foam dressing can be done in prominent areas to prevent ulcer development)
*use of positioning devices can be used to prevent unnecessary friction and shear while sliding
*reduce dragging the patient, it can cause friction rub
*provide atleast second hourly position change and backcare or massage .
