In healthcare system accidental falls for patients is dangerous. Patient injury or death occuring as a result of falling.

Steps

1. Explain the components of fall prevention plan and its purpose

2. Provide emotional support as needed

3. Assess the patient for risk for falls using a facility approved fall risk assesment tool.

4 . Educate the family members as necessary to gather information about patients medical history and personal habits . Include the use of medication and diet plan.

5. Follow facility protocols for choosing appropriate strategies that will reduce fall risk based on the individual risk assessment.

6. Place a label or sign on the door of the patients room. Collaborate with other multidisciplinary team to implement the fall prevention.

7. Keep the bed in lowest position and lock the wheels. Keep the side rails up when no one is in attendance.

8. Identify and modify environmental hazards example , clutter , wet floors .

6. Place a label or sign infront the patient room on the headboard of the patients bed, and or in the cover if the patients medical records , indicated by the facility protocols.

7.keeping the bed in lowest position is to patient comfort and avoid the falls from high position . By locking the wheels and put up the side rails also help to prevent falls .

8. Identify the environmental hazards like wet floor . It should be help to prevent falls.
