1) when we document the child's immunization in their chart, what is the essential information we document?
2) Hepatitis B vaccine can come in a Vial or a Syringe. What is the difference?

Finally information about this has to be given to the state / government immunization information center.
Hepatitis B vaccine
Syringe: syringes will suck desinated amount of medication from the ampoule.
Vials: They have rubber stoppers and put needle into it and take medication. There are single dose vials and multidose. In case of multidose vial upto 10 doses are present , once after taking each dose properly stored in refrigerator.
ENGERIX_B is available both in vials and prefilled syringes.

Prefilled syringes:
In case of Hepatitis B vaccine available in 10 disposable prefilled Tip-Lok syringes means luer-lok needles (10mcg/0.5ml) Trade name: ENGERIX-B
For the prefilled syringes attach a sterile needle and administer directly to the patient intramuscularly. The amount of vaccine for one dose is already present in prefilled syringe.
Accurate and correct amount of vaccine is administerd in this one without any wastage of drug.
Single use, after use you can throw them.
VIALS:
In case of vials,sterile needle is used to suck medication dose and given IM.
Sometimes a little bit of vaccine is sucked more or less from the vial and while withdrawing little vaccine may be wasted.
If multidose vials , they have to be properly stored.
