Electronic Health Record (EHR)
An electronic health record (EHR) or electronic medical record (EMR) is the systematized collection of patient and population electronically-stored health information in a digital format. EHR can improve quality care by using the data and analytics to prevent hospitalizations among high - risk patients.
1)Definition of EHR (Electronic health record ) : The portions off a patient's medical records that are stored in a computer system , aka electronic medical records , computerized patient records, or electronic chart.
2) Advantage of codified data over document imaged data : When EHR data coded, it can be used for trending, alerts, health maintenance , decision support, it can be accurately identified and electronically compared by computer. Document image data requires a human being to read the information.

The electronic signature is a computer process that has :

c) User authentication - the recipient must be able to confirm that the signature was in fact signed by real person.
