geriatric nurse or gerontological nurse is a type of nurse that helps care for aging and elderly individuals. They are trained to be able to perform traditional nursing duties, yet they also have special training that helps them better understand the special needs of many elderly people.
The geriatric assessment is a multidimensional, multidisciplinary assessment designed to evaluate an older person's functional ability, physical health, cognition and mental health, and socioenvironmental circumstances. It includes an extensive review of prescription and over the counter drugs, vitamins, and herbal products, as well as a review of immunization status. This assessment aids in the diagnosis of medical conditions; development of treatment and follow-up plans; coordination of management of care; and evaluation of long term care needs and optimal placement.
Because of the demands of a busy clinical practice, most geriatric assessments tend to be less comprehensive and more problem-directed. For older patients with many concerns, the use of a “rolling” assessment over several visits should be considered. The rolling assessment targets at least one domain for screening during each office visit. Patient-driven assessment instruments are also popular. Having patients complete questionnaires and perform specific tasks not only saves time, but also provides useful insight into their motivation and cognitive ability.
As assessment data are obtained, they need to be recorded to allow all members of the health care team to easily access the information. The family physician can generate a problem list that includes any condition or event requiring new or ongoing care; the medical, nutritional, functional, and social implications; and proposed interventions. This type of assessment allows older patients to benefit from an interdisciplinary team that is effectively assessing and actively managing their health care.

When I talked to an older patient with slight depression. He told that he need spiritual need. Spiritual needs are defined as needs and expectations which humans have to find meaning, purpose, and value in their life. such needs can be specifically religious but even people who have no religious faith or are not the members of an organized religion have belief systems that give their lives meaning and purpose.
Then I supporter him with that feeling and talked with him about that topic like I too have a spiritual and socio cultural beliefs. So that the elderly patient can feel comfortable with me and explains his disease completely.

Once I met a older patient. I asked him to tell about himself to examine the social state. He hesitated to tell me because he is nothing special. Then I understood that I should encourage him to keep his own identity. I focus on a person's positive attributes.reduced the embarrassment.validated the person's sense of I became his biggest Fan. As your child develops interests encourage them by showing interest in the things they love. That how it works.

When met an older patient with gastric problem. I asked him the food preferences. He didn't tell me anything specifically because he told me that at this age i am not demanding anything. I am just living. Then I understood that he need encouragement. So I started motivating him by saying one should have preferences whether it happens or not.
