The nursing diagnosis is: post-circumcision care for a newborn relating to pain reduction from incision, bathing hygiene, and administering Tylenol as evident by the mothers statement on pain, hygiene, and and nursing assessment of surgically circumcised newborn.
desired outcomes: Patient will be able to identify 3 soothing techniques if baby is uncomfortable following procedure by the end of teaching.
Based on the nursing diagnosis and desired outcomes mentioned above, what would be your teaching plan content/ nursing action for the mom of baby? please be very specific.
Teaching plan content/ nursing action for the mom of baby are as follows to sooth the baby when in uncomfortable situation:
1)Caring for a circumcised ares after the circumcision, you can comfort your baby by holding him and nursing him often.
The circumcised will take 7 to 10 days to heal. The area may be red for a few days and you may see some yellow discharge, which should decrease as it heals.
2)Follow the instructions given by the practitioner who did the circumcision about caring for the dressing, using petroleum jelly, keeping the area clean and bathing hygiene, and administering Tylenol as evident by the mothers.
3)You may see persistent bleeding at any time during the healing process.The redness and swelling around the circumcision do not start to go down in 48 hours.
Your baby seems to be unwell.
Your baby does not pass urine within 12 hours of the procedure.
There is a greenish or foul smelling discharge from the curcumcised area.
if these all problems arises in that case you have to visit nurse practitioner.
#Teaching plan for mom of a baby are:
1)Check for bleeding
Blood staining of the diaper is common after circumcision. During the first 24 hours, check your child's diaper at every diaper change for active bleeding.
2)Ease swelling
Bathe your child frequently after circumcision, your infant should have a warm bath every day for one week.
3)Prevent skin sticking
Gently push back the edges of the circumcised skin from the glans after the first two weeks of healing. This will prevent the skin from sticking to the glans as healing continues.
4)Apply a barrier ointment
Apply a topical barrier ointment (such as petroleum jelly) on the end of the circumcised area after every bath and with each diaper change until the circumsed part has healed completely. these are all the plan nurse has to teach to a mom of the baby.
