The nursing diagnosis is: post-circumcision care for a newborn relating to pain reduction from incision, bathing hygiene, and administering Tylenol as evident by the mothers statement on pain, hygiene, and and nursing assessment of surgically circumcised newborn.
desired outcomes: Patient will be able to state 3 steps of circumcision care of their newborn prior to discharge
Based on the nursing diagnosis and desired outcomes mentioned above, what would be your teaching plan content/ nursing action for the mom of the newborn baby? please be very specific.
1st step:- pain reduction from incision
- infant may feel pain after circumcision. It usually do not lasts more than 48-72 hours.
- signs of pain include crying, feeding difficulties..etc
- for pain relief, acetaminophen can be given.
Acetaminophen- oral- 10-15mg/kg/dose 4-6 hourly as needed.( do not exceed 5 doses per 24 hours)
2nd step:- Bathing hygiene
- usually bathing can be started from 3rd day of surgery. Here, it is a newborn baby. so wait until umbilical cord Fall off( 14 days)
- till then, use sponge bath to clean infant and his penis. use warm water. avoid any kind of soap, avoid very warm water.
- When bathing starts, use warm water and avoid soaps which may cause irritation
- on diaper change, when you notice stool on incision site, clean with mild soap and water.
- Apply a barrier ointment (petroleum jelly) at the incision site after each bath and diaper change
3rd step:- Application of tylenol
Tylenol PO- 10-15mg/kg/dose 4-6 hourly as needed(dont exceed more than 5 doses in one day)
