The Concept of Managing Care
Mr. Montoya is to be discharged from the acute care facility after removal of a metastatic brain tumor. He will receive chemotherapy intrathecally as well as intravenously. The unit clerk has scheduled follow-up appointments with his surgeon, family provider, oncologist, and neurologist as well as with the infusion therapy department. The physician has provided referrals for consultation with a dietitian, social services, and home nursing care.
Linking the concept of managing care with the concept of collaboration:

How can the nurse prioritize care to meet the client’s needs while ensuring that each of these services meets her own needs in preparing for the client’s discharge (e.g., nutritional screening predischarge, social services interview, and home nursing assessment to determine home care needs)?

1)The nurse should prioritise the home nursing care services checking out who will look after the client in home for medication .
Nutrition plays an important role because the patient develop poor feeding quality in food,lack of appetite, intolerance to certain foods.Nutrional screening helps us to know the required amount of calories requirements per day and how to make out.
Social service department has licensed social worker who could help him to cope up with the impact of diagnosing cancer.
2)The main nurses role is providing psychological support to family and patient.Enabling the administration of medication in right way.She has to collaborate with the entire health care team while discharging the patient from hospital to ensure patient is instructed for follow up,inform any side effects immediately
