Antidote for heparin is protamine sulfate, which will neutralize heparin or stop heparin from working. The amount of protamine sulfate is based on the heparin dose given in the last two hours. Antidote for warfarin is vitamin K, which will reverse high INR values. Daily aPTT (activated partial thromboplastin time) monitoring test is done for heparin and PT/INR monitoring is done for warfarin.
