1) what is mg/ hr for this patient.?
In the question, it is given that infusion rate is 7.5 mg/kg/hr means we have to give 7.5 mg per kg body weight for each hour.
Here, weight of patient is 50 kg
so, the dosage to be given for this patient per hr can be calculated as
7.5 mg/kg/hr × 50 kg
= 375 mg/hr.
2) The pump will be set at .....ml/hr.?
The pharmacy sends vancomycin 750 mg in 250 ml of D5W.
Which means 750 mg vancomycin is present in 250 ml D5W
For this patient we have to give 375 mg vancomycin per 1 hr
so, we have to calculate how much fluid contain 375 mg vancomycin.!
Let us assume 375 mg vancomycin is present in 'X' ml
750 mg ➡️ 250 ml D5W
375 mg ➡️X ml D5W
so, X = 375 mg × 250 ml ÷ 750 mg
= 125 ml
So,375 mg is present in 125 ml
As we have to infuse 375 mg/hr and 375 mg is present in 125 ml infusion pump should be set at 125 ml/hr.
3)How long will the infusion last.?
The infusion will last for 2 hrs.
( As 125 ml is infused in 1 hr, 250 ml will be infused in 2hrs)
