A patient/client weighs 200 lb. Available is 25,000 units of heparin in 1000 mL of D5W. The physician orders the following: heparin sodium IV bolus of 80 units/kg and start the drip at 14 units/kg/hour. What is the correct IV rate in milliliters per hour? Round to the nearest whole number.

Calculation:-
Step 1 : Weight of the patient = 200 lb = 90.7 kg
Step 2 : 1000 ml of D5W contains 25000 U of heparin.
Therefore , 1 ml contains 25000 / 1000 =25 U of heparin.
Step 3 : Dose prescribed by physician = 80U / kg = 80 90.7 = 7256 U
Rate prescribed = 14 U /kg/hour = 14 90.7 U / hour = 1269.8 U / hour = 1270 U /hour (approx).
Therefore, we have to give heparin at the rate of 1270 U/ hour.
Step 4 : From step 2, we know 25 U of heparin is present in 1ml of D5W.
Therefore ,1U of heparin is present in 1/25 ml of D5W.
1270 U of heparin is present in (1/25) 1270 = 50.8 ml = 51 ml (approximate) of D5W.
