A patient is ordered for Heparin 1,800 units/hr IV infusion. Heparin is available in the following concentration: Heparin 25,000 units mixed in 250 mL D5W. Calculate the rate in mL/hr. _______ mL/hr.

ml/hr = Volume(ml) / Time(hr).
Calculation of volume(ml).
Volume(ml) = Desire dose/Dose in hand * Quantity.
Here
Desired dose = 1800 unit.
Dose in hand = 25000 unit.
Quantity = 250 ml.
As per above formula
Volume(ml) = 1800/25000 * 250
= 0.072 * 250
= 18 ml.
Time(hr) = 1 hr.
So
Rate(ml/hr) = 18 ml/hr.
