EHR ( electronic health record) system records all the patients Information regarding diagnosis, treatment as well as medical history. The accurate and complete documentation in the electronic health record is important for the reimbursement and quality care as the healthcare insurance company requires all the data and the information regarding patient's treatment that should be correct and accurate for the reimbursement. They have certain types of codes and policies which is used for reimbursement. If the codes and the information doesn't match then the healthcare insurance company may deny the reimbursement.

The most important thing regarding accuracy as it provides a platform for providing quality of care to the patient by ensuring the Communications between healthcare provider and patient. The information provides base for the healthcare professionals to provide better treatment services by monitoring the medical history, treatment and diagnosis records saved in the EHR.
