HEALTH ASSESSMENT :

Health assessment is a systematic and continuous validation of an individual it includes the individuals perceived needs, health problems, health practices, values and lifestyles,

We can relate the assessment needs to the chip program of Bergen County to identify the magnitude of the problem based on two criteria

FIRST CRITERIA:

.To assess the number of people affected

. The degree of health issue that leads to death or disability, if the health issue impairs quality of life

SECOND CRITERIA :

. To measure the positive impact the local health departments, hospitals and community organisation on health.

The focus areas of assessment are as follows

1. Obesity, fitness, the importance of nutrition and the prevention as treatment of chronic disease.

2. The importance of mental health and how it plays a major role in being healthy, and prevention of substance abuse, it's effects on mental health

3. Access to health care, and treatment strategies.

SETTING GOALS AND OBJECTIVES INTERVENTIONS AND EVALUATION

The vision of the community health improvement partnership (CHIP) is that the people in bergen county should have access to all the resources that might enable them to reach an optimum level of health

1. GOALS RELATED TO OBESITY, FITNESS NUTRITION AND CHRONIC DISEASE:

The following are the goals related to obesity , fitness ,nutrition and disease.

GOAL 1. the major factor to target obesity and reduce the risk is to increase physical activity throughout the community.

To provide free or low-cost exercise opportunities in the parks and selected locations in the community

GOAL 2. to increase healthy eating through multiple educational programs,

conducting various nutrition programs to enhance the knowledge of a well-balanced diet.

GOAL 3. To conduct educational programs

To enhance knowledge of certain chronic diseases and it's management.

GOAL 4. To increase awareness of end of life care and palliative care.

To provide education in the community and collaborate with the local hospitals to increases access to care.

INTERVENTION

To enhance participation and motivate the members to take part in the program.

To detect the number of community members joining the challenge.

EVALUATION

Evaluation is done by a method of survey. The detect the number of participants and the effectiveness of the program, to detect if the goal is achieved and the community members are benefited by the educational programs

2. GOALS RELATED TO MENTAL HEALTH AND SUBSTANCE ABUSE

The following are the goals related to mental health and substance abuse

GOAL 1. To reduce depression and isolation

Provide education and psychological support to those who are prone to depression and to collaborate with the hospital to reduce depression in the community

GOAL 2. To reduce anxiety and stress.

Anxiety and stress are commonly seen in the community due to the environmental stressors hence educational outreach is essential to reduce anxiety and stress.

GOAL 3. Reduce stigma related to stress

Stress is a common factor that affects mental health, ways to reduce stress as burnout is communicated to the community members

GOAL 4. Reduce alcoholic binge drinkers

To increase the awareness of the harmful effects of alcohol and the risk associated with it.

GOAL 5. Reduce the prescription of drugs abuse

To increase the awareness of the harmful effects of drugs to mental health

GOAL 6. To promote assess to health care.

Psychological support and psychotherapy should be made easily available to the members of the community

INTERVENTIONS :

To enhance the participation and the community in the educations and prevention programs

To increase the number in collaboration programs.

To survey and monitor alcohol use

EVALUATION:

Evaluation is a method to check if the programs conducted has an impact on the mental health of an individual. To determine if the programs were effective in reducing the risk related to mental illness.

GOAL 3. Related to access to care.

The following are the goals related to access to care

Goal 1. Promote disease identification and prevention

To enhance programmes related to immunizations, education and outreach to the community which offers screening to the community to detect any diseases and immediate measures to prevent the diseases.

GOAL 2. to promote care coordination.

To collaborate with the local hospital to promote care and enhance the quality of life.

INTERVENTION

To enhance the participation of the community in the various programs and increase the number of screenings and the educational programs

EVALUATION.

To determine if the goals are achieved and if they were effective to promote care coordination and engagement in primary care.
