Capillary refill time (CRT) is the time taken for colour to return to a distal capillary bed after pressure is applied to cause blanching. CRT is regarded as the quickest and easiest way of assessing early hypoperfusion.
It is usually measured by holding a hand higher than level of heart and applying pressure on the nailbed until it turns white; then recording the time needed for the colour to return once pressure is released.
Normal capillary refill time is usually less than 2 seconds.
In newborn infants, capillary refill time can be measured by applying pressure over the sternum for five seconds with a finger, and noting the time needed for the refill. The normal capillary refill time in newborns is less than 3 seconds.
Capillary refill time is a marker of peripheral perfusion.
CRT is prolonged in cases of :-
1) Circulatory shock- Dehydration, Sepsis , Dengue hemorrhagic fever
2) Cold extremities
3)Peripheral vascular disease
