The Subjective, Objective, Assessment and Plan (SOAP) chart is a commonly used technique used by health care professionals for doing documentation in a structured and systematic way. Review of Systems (ROS) is a system centered list of questions that helps to identify symptoms not else stated by the patient. In the aforementioned patient’s case, ROS includes the following
