treatment of cancer cervix depends on the stage of the disease
FIGO staging of cancer cervix:
stage1: tumor confined to cervix only
1A:diagnosed only by microscopy, invasion <5mm depth and <7mm horizontal spread
1A1: invasion <3mm depth, <7mm horizontal spread
1A2: invasion 3-5 mm depth, <7mm horizontal spread
1B: clinically visible tumour, confined to cervix, invasion greater thanthat of 1A
1B1: < 4cm in greatest dimension
1B2: > 4cm in greatest dimension
stage 2: tumor invades beyond uterus but not to the pelvic wall or to the lower third of v agina
2A: without parametrial involvement
2B: with parametrial invasion
stage 3: tumor involves lower third of v agina, extends to pelvic wall, causes hydronephros or non-functioning kidney
3A: tumor involves lower third of v agina
3B: tumor extends to pelvic wall or causing hydronephrosis
stage 4: tumor invades mucosa of the bladder or rectum or extends beyond true pelvis or distant metastasis
treatment of cancer cervix:
1A1: cone biopsy or trachelectomy or simple hysterectomy
1A2: radical hysterectomy, simple hysterectomy or trachelectomy+ pelvic lymph node dissection
1B, 2A: radical hysterectomy+ pelvic lymph node dissection+/- para aortic lymph node dissection or chemoradiotherapy
2B, 3, 4A: neo adjuvant chemotherapy followed by surgery or radiotherapy
4B: chemo radiotherapy
