Conduct a health history on a family member or friend. Be sure they give you permission. Using the interviewing techniques learned in Module 2, gather the following information. Use your textbook as your guide.

  • Present Health
  • Past Health
  • Family History

While this is only a partial health history, summarize in 3 -5 pages the information you gathered. Also, answer the following questions:

  1. Was the person willing to share the information? If they were not, what did you do to encourage them?
  2. Was there any part of the interview that was more challenging? If so, what part and how did you deal with it?
  3. How comfortable were you taking a health history?
  4. What interviewing techniques did you use? Were there any that were difficult and if so, how did you overcome the difficulty?
  5. Now that you have taken a health history discuss how this information can assist the nurse in determining the health status of a client.

PAST HISTORY

History of hapatitis B infection 6 years back

History of blood transfusion following loss of excessive blood after injury to hand 7 years back.

History of dengue fever 10 years back

Diabetic, hypertensive and hyperlipidemic

FAMILY HISTORY

Both father and mother were diabetic, hypertensive and hyperlipidemic.

Mother had breast cancer and undergone breast removal surgery.

Brother is diabetic

  1. Patient was willing to share information.
  2. Asking past medical history was challenging because patient cannot recollect all the past medical conditions. So i also asked her son about her past medical history.
  3. Asking all her medical conditions , and the medication taken was a difficult task. But she co-oparated well . so i was comfortable interviewing her.
  4. It was an informative and problem oriented interview.
  5. The difficulty in interview is overcomed by making patient more comfortable,and by not asking too complicated and confusing questions, showing rapport, empathy and support. Giving RESPECT to the patient was the best part of interview.
  6. A proper medical history will help the nurse to provide most appropriate and effective treatment to the patient. Taking history will establish good rapport to the patient and her family, this will make environment more comfortable. It will also provide information about patient's current health status and current concern.
