How do your healthcare providers (e.g., hospital, emergency department, personal physician) keep you safe from being harmed by the effects of healthcare services? What could your providers do better to keep you safe? Provide references

Healthcare providers or units are committed to provide safe and secure environment to patient, if any failure occur wheater it is intentional or unintentional it affect the repot of the health providers. Intentional problems are most of the time correctable but unintentional are the one which occur due to lack of knowledge, laziness, communication gap and unorganized unit.

The patient safety is depending on right treatment, prevention of falls, prevention of medication error and hospital acquired infection. It may harm the Patient and increase the stay of patient in hospital for long and our focus for effective management of the patient disease not able to achieve.

Health care units have responsibility to promote and achieve the health standard

Measure taken by providers to promote client safety.

1. Hospital acquire infection

- Washing hand before or after any procedure

- Maintain sepsis while performing any procedure.

- Separation of infectious patient from other stable patient.

- Catheter care and iv canula care according to hospital protocol.

- Limitation of visitor.

- Disinfection of the surfaces

- Prevention of patient to walk bare foot

- Avoidance of invasive procedure if not required.

2. Falls

- Keep the call button nearby and person items

- Provide safety companions (family member, siderails etc)

- Do safety round.

- Provide waling aid if required.

- Don’t off light in night. Make it dim

- Telling patient about medication making client dizzy.

3. Medication error

- Asking identification of the client.

- Checking of dosage and route before giving medication

- Avoidance of herbal supplements.

- Don’t interfere with doctor prescription.

4. Pressure sore

This usually occur when client is immobile for long time, secondary to the negligence of the health care provider. The high-risk patient needs to be changed position every 2 hourly, comfortable devices, adequate nutrition.

What could your providers do better to keep you safe

There is no bar for the standard of care, but some measures a provider should take to keep a client safe and healthy in the healthcare setting. These implementations are :

- provide adequate knowledge about patient treatment

- assure the client by replying all the queries they have

- tell them about the do’s and don’t related to disease like important of carbs before or after exercise in diabetes,

- explain about monitor alert related to disease to relatives.

- Provide information while giving injection and medication.

- Tell family member about importance of handwash

- Ongoing health education related to patient care.

- Explain the importance of comfort devices.

- Write information or pamphlet like diet plan, health education
