• Chief complain- Patient is suffering from shortness of breathing and weight loss.
• History of present illness- patient feels comfortable after providing oxygenation but her activity level is hampered.
• History of past illness- Patient was admitted in this hospital for shortness of breathing for three times in past year.
b. The conditions are.
1) Impaired gas exchange related to alveolar hypoventilation.
2) Imbalance nutritional status less than body requirements related to improper food intake as evidenced by weight loss.
3) Activity intolerance related to shortness of breathing.
c. The conditions are.
1) Patient may suffering from chronic obstructive pulmonary disease ( As weight loss occurs along with dyspnea.)

•Collaborative diagnosis helps to determine the main symptomatic problem of the patient.
• Nursing diagnosis helps to determine the multiple symptomatic problems of the patient related to the particular disease condition.
