Malik Reynolds is a 12-year-old African American male who comes to the clinic with his father, James, for a routine health visit. Malik is 5 ft 2 in tall and weighs 175 lb. A routine urinalysis reveals glucosuria with negative protein, WBC’s and Blood. A routine CBC shows WBC’s 18.1 RBC’s 12.24, HCT 38.2, Hgb 15.8. His vital signs are: HR 72, RR 18, Temp 98 degrees F, BP 122/86.
1 . Malik is overweight for his height and age. His WBC levels are elevated and there is presence of glucose in his urine. These are the symptoms that has to be reported to the provider.
2. The questions that has to be asked to Malik are :
a. Are you experiencing increased thirst ?
b. Do you pass urine frequently ?
c. Has your hunger increased recently ?
d. Do you feel tired all of a sudden?
All these questions are asked to rule out the symptoms of diabetes.
3. Objective symptoms that can be seen :
a. Dry and itchy skin
b. Darkened skin patches usually noticed in the creases of the body.
4. Fasting blood sugar, Random blood sugar, HbA1c to rule out diabetes.
5. Health Education :
a. Eat small frequent meals - Prevents spike in blood sugar levels.
b. Avoid junk foods - avoid unnecessary calories.
c. Lead active lifestyle - helps in managing blood sugar levels.
d. Take medication as prescribed - Helps in decreasing the symptoms and leading a normal life.
e. Ensure regular follow up - determines the health status of Malik and helps to identify the issue soon.
6. Nurses note :
Malik is examined . He is found to be overweight and there is presence of sugar in urine and WBC reveal infection. Patient and his father is educated regarding blood investigations to be done and healthy lifestyle options to be followed.
