Write a letter to your insurance company.
A child was hurt at your business. She fell down and broke her arm, according to the doctor's report from the hospital she visited after the accident.
It should have the details they need to know about the event so they can as follow up on a claim. Details can include an eyewitness view (either your account or that of someone else who saw what happened). You should also include when and where in the business it happened. Note anything the parent or guardian of the child said about the event or the injury, if there was anything they did to cause the event or anything they should have done to prevent it. If there is something the business did that led to the accident then be honest and describe it (e.g., the aisle was mopped a few moments before, which caused the running child to slip).
The incident report may be used in litigation so you will want to describe all the necessary details with factual information, not opinions. If you do not have information for some part of the story, write that down. Include names of people involved, any ID numbers if appropriate, times, locations, etc. You can write someone's opinion, but label it as such.

Sub: Intimation of a child hurt incident in our business premise yesterday evening.
Dear Adam,
Hope this letter finds you all well.
I would like to bring to your notice an incident which happened in our business premise yesterday evening wherein a child was hurt as she fell down in the aisle near the billing counters. Later after the medical test it was revealed that she had broken her arm and will be requiring few weeks rest to recover.
The unfortunate event happened yesterday, wherein Mr. Ajay (duty supervisor) employee id-1234 who was talking to the cashier on duty saw the complete events unfold as an eyewitness and described the unfortunate even as something which could have been avoided.
As the Aisle is a long passageway kind of area along with the billing counters some of the kinds coming in to the store tend to run and slide on the marbled floor. We already have signs installed there notifying parents to keep kinds along all the time while in aisle, however in this unfortunate case as the parents of the kid were busy checking some items behind the counter the said kid started to run around in aisle and sliding on the marble floor (unsupervised) While she was doing so her shoe sole suddenly came off thereby breaking her run and making her fall flat on her left arm. Guard on duty Mr. Rick was nearest to the place where she fell and immediately reached out and tried to gauge and assist in the situation. The kid’s parents also had arrived in the meanwhile and picked up the kid who was then taken to the branch manager room and given some first aid. As the kid was in unbearable pain branch manager Mrs. Jonas quickly intimated 911 and requested for an ambulance which responded promptly in time and Kid was taken to the hospital for further diagnose.
Our Branch manager Mrs. Jonas (employee id-6789) also accompanied the parents and provided a helping hand to best of her abilities. While she was taking their leave after diagnose, parents of the girl did acknowledged that they should have been more careful in leaving the kid unsupervised in the aisle area and thanked her and entire store employees for their quick support and helping attitude.
