Mr. Robert McCelland, an 81 year old ,is new admission from local hospital to your long term care facility. After Mr.McClelland's last about with pneumonia and cogestive Heart Failure ( CHF) , his wife of 59 years has decided she is no longer able to care for him at home. Mr. McClelland states ,''He just gotten too weak and can't help me care for him . I am afraid he will fall and hurt himself. I am worn out trying to care for him myself. I have to bathe him and remind him to eat. He can be so forgetful. I hope am making the right decision for him because he never want to go into nursing home "
You will use above scenario ( case study ) two write two actual nursing diagnoses and one risk for nursing diagnosis. Next you write what you will implement for nursing diagnosis . Then you will state whether your goals have been met or not and then reevaluate what is needed.
The nurse offers a diagnosis for a problem present during the nursing assessment which is caused by the disease or disorder . A doctor will diagnose a patient with CHF and prescribe medication . A nursing diagnosis is not simply assessing the patient , which is first step in the nursing process.
To investigate the association of nursing diagnosis identified for CHF based on their hemodynamic profile. The main nursing diagnosis were risk of infection , bathinhbself care deficit , risk for decreased cardiac output, risk for falls and excess fluid volume . There was no statistically significant difference hemodynamic profile and the nursing diagnosis found.
Nurses need to develop skills for improving their diagnostic abilities to ensure implementation of consistent interventions. In order to select the correct nursing diagnosis , the nurse must make quick and accurate conclusion from patient data during assessment. The diagnosis phase is a critical step as it is used to determine the course of treatment.
The implementing phase is where the nurse follows through on the decided plan of action. Actions involved in nursing care plan include monitoring the patient for signs of change or improvement, directly caring for the patient or performing necessary medical tasks, educating and instructing the patient.
Every nursing diagnosis will have a goal of rvaluating outcomes . If the goal is measurable , you will be able to state whether the goal was met , not met or partially met a critical appraisal or assessment , a judgement of the value , worth , character or effectiveness of something measurements of progress .
There fore the purpose of the evaluation is to determine whether outcome criteria have been met and how care for the patient might be improved. Nurses are well placed to evaluate their services and practice and to develop an evidence base for effective care delivery.
