Mr. William, 85 years old male client is living in a long-term
care facility. He is having progressive dementia over the past
seven years and is unable to care for himself independently due to
cognitive decline. He has got visual and hearing impairment.
Identify actual and potential nursing interventions to improve the
self-care ability of Mr. William in relation to the following
cognitive decline and
sensory deficits in 50-60 words)
1) Self care deficit bathing/ Hygiene related to dementia
Interventions
- Assess the patient's appearance
- Keep all articles ready in the bathroom
- Allow Mr William to perform activities whichever are possible.
- Check the skin condition before and after bath.
2) Disturbed sensory perception related to dementia
Interventions
- Assess listening and seeing ability
- Provide reading material with large fond
- Provide adequate lightning at night
- promote use of hearing aids
- provide magnifying glass
- instill ear and eye medications as prescribed.
- encourage participation in physical and social events, it prevents isolation.
- provide assistance in activities of daily living and while ambulating.
3) Risk for injury related to impaired decision making and disorientation and decrease in eyesight
Interventions
-Assess the degree of impaired ability
- Assess patients surrounding for hazards and remove them
- Divert attention to Mr William when agitated or restless
- maintain adequate lightnenil and clear pathway.
- Instruct relatives to double lock doors and windows.
- Assess level of cognitive disorder.
