Identify which process analysis tool is best for identifying the potential causes of a problem and briefly explain how it identifies causes. Using that information give an example of a problem and explain the method of identifying the cause.

The process analysis tool that is best for identifying the potential causes of a problem is; fishbone analysis tool. The team using the fishbone diagram tool should perform the following steps; Agree on the problem statement o referred to as the effect. This should be written at the mouth of the “fish.” One should be as clear and specific as one can about the problem. Agreeing on the major categories of causes of the problem that are written as branches from the main arrow. Major categories should include equipment or supply factors, environmental factors, rules/policy/procedure factors, and people/staff factors. Brainstorm all the possible causes of the problem. Asking “Why does the problem happen?” Once each idea is given, the facilitator should write the causal factor as a branch from the appropriate category and then place it on the fishbone diagram. Causes may be written in several places if they relate to several categories. ● Again asking “Why does it happen?”. Writing sub-causes branching off the cause branches. ● Continuing to ask “Why?” and generating deeper levels of causes and continuing the organization under related causes or categories. This will help in identifying and then addressing root causes to prevent future problems.
For example, we may discuss a problem like lack of communication in our work. Then we may do the root cause analysis; the difference in culture, personality issues and language barriers. Then a brainstorming about how to address this issue; for example by having group discussions and more socialization in the team.
