The developmental age group of toddler 1 to 3 years exerts a considerable influence on all other family members, regardless of the size and form of the family unit. The physiologic functioning of the body is mature by the end of toddler period. The physical assessment includes inspection, palpitation, percussion, and auscultation. The complete assessment includes the primary assessment and the focussed assessment.

Primary assessment includes the ABC (Airway, breathing, and circulation).

  • Skin: Colour of the skin, turgor, presence of any lesions, bruising, wounds or injuries.
  • Hydration status: Assess hydration and nutrition status and check feeding type- oral, type of feeding, breastfeeding.
  • Output: Assess Bowel and Bladder habits, incontinence, toilet training. Maintain fluid balance activity
  • Focused Assessment: which includes the present problem or other identified issues, e.g. cardiovascular, respiratory, gastrointestinal, genitourinary, immune system and nervous system.
  • Risk Assessment: pressure injury risk assessment, falls risk assessment, accidental injuries such as drowning, burns, suffocation, poisoning.
  • cognitive development: the toddler who plays with the various type of toys learns to recognize shapes, sizes, and textures of play materials.
  • Moral Development: Understanding of the moral values, and ability to differentiate the right and wrong things.
  • Language and Speech development: The child talks to the parents and the other persons as well as the toys.
  • Assess for the sleeping habits, emotional status, and coping strategies.

Health Promotion:

The nutritional intake for toddlers include the essential nutrients, in the amount necessary for maintenance, replacement, and growth of tissue and for energy. The daily caloric requirement is 100Kcal per Kg during the toddler period. The reasonable amount of walking, climbing, and exploring increases the appetite. Toddlers may have the short attention span and become restless which precipitates the temper tantrum. The toddler has the sense of autonomy and the parents should not force them which result in negativism. The child has to be given freedom for the type of food and kind of food. Accidents are the largest single cause of mortality and disability between the ages 1 and 4 years. The parents have to understand the child level of development and try to avoid the hazardous situations.
