For each interaction, describe the conversations that could take place between the patient and individual. For example, the Nurse-Patient interaction would consist of the Nurse obtaining the patient’s chief complaint, vital signs, etc. please provide references if available

A conversation between the patient and nurse while taking medical history.

Obtain chief complaints: Nurse should collect the patients chief complaints, for example if he complains about abdominal pain, we need ask them what is the location, intensity, type of pain (such as pinching, twisting, vibrating), duration of pain. Also ask about his bowel movements whether it regular or irregular, if he says irregular and constipation, check for any Melina and pain.

Nurse: Hello sir/madam

Patient: Hi, I have abdominal pain since two days

Nurse: oh, Show me the location where exactly you are getting pain..? and type of pain

Patient: : I have pricking type of pain Right upper quadrant, and also I have constipation problem.

Nurse: Do you have any burning sensation in the chest..?

           About constipation – do you have hard stool and pain at the rectal outlet during defecation..?

Patient: : No burning sensation in the chest, yes it is very hard to pass the stool, I get a severe pain at rectal outlet

Nurse: Did you see bleeding in the stool…or black color stools..?

Patient: : I don’t see black color stools but yes streaks of blood will be present.

Medical History: Nurse should collect their medication history, allergic history, past and present medical history also.

Past medical history: you need collect whether he had any recent medical/surgical illness

Present medical history/medication history: Ask if he has any medical problem for which he is using medication. For example Thyroid- taking 20mg of Thyronorm.

Nurse: Are you allergic to any food or medication?

Patient: : No

Nurse: Have you undergone any surgeries in the pas..?

Patient: : Yes, I underwent appendectomy a couple of years ago

Nurse: Does your parents have HTN, DM or any cancers..?

Patient: : No

Vital Signs:

Nurse: I would like to check your BP, pulse, respiration and temperature. Then I will send you to meet Dr.XXX

Patient: : Okay, Alright
