The nurse is working with a new client at the community clinic. The client is a 59-year-old African American female. The client's mother was diagnosed with coronary artery disease at the age of 60. The client has a sedentary job but walks for 30 minutes every day on the weekend. Her waist cirumference is 38 inches (96.52cm). Her blood pressure on this visit is 140/90 mm Hg. She stopped smoking six months ago.

Laboratory results are:

   Triglycerides 250 mg/dL

   HDL 445 mg/dL

   LDL 150 mg/dL

   Total cholesterol 300 mg/dL

The nurse establishes a NANDA-1 nursing diagnosis of Deficient Knowledge related to coronary artery disease.

Based on the above information and in 100 words or more, please do the following:

Develop a teaching plan that addresses two of the client's risk factors for coronary artery disease.

The plan should consider cultural influences and must include the following:

1. a measurable patient centered outcome

2. three (3) nursing interventions to help the client achieve the outcome

3. an explanation of how each intervention will help the client achieve the outcome

Risk factors for coronary artery disease include dyslipidemia,smoking,hypertension,gender ,ageing etc.

Modifiable risk factors are:

• cigarette smoking
• eleveted blood pressure
• high cholestrol levels
• obesity
• physical inactivity
• use of oral contraceptives
• stress etc

Non modifiable risk factors:

• Positive family history
• age,gender,race etc

Desired outcomes :

• Report anginal episodes decreased in frequency,duration and severity
• demonstrate relief of pain as evidence by stable vital signs,absence of muscle tension and restlessness.

Nursing interventions:

• Assess and document patient response to medication:providing information about disease progression,aids in evaluating effectiveness of interventions and may indicate change in therapeutics regimen.
• Identify precipitating event,if any frequency ,duration,intensity and location of pain
• Observe for associated symptoms dyspnea,nausea and vomiting,dizziness,palpitations etc.
• Monitoring heart rate and rhythm,monitor vital signs for every 5min during intial anginal attack,provide supplemental oxygen as indicated.
• Evaluate reports of pain in jaw,neck and shoulder,arm or hand as cardiac pain may radiate towards these organs and referred to more superficial sites served by the same spinal cord nerve level.
