Joyce Workman, 42- year old female who presents to the Diabetes Clinic with a new diagnosis of type II diabetes. She has been documented as being obese, new onset hypertension, polyuria, and a rash on her abdomen. She was asymptomatic upon arrival. She was admitted yesterday for stabilization of her glucose levels, and assist her with lifestyle modification. She states she leads a sedentary lifestyle as a bank officer. Her HbA1C is 10%.

Mrs. Workman calls the nurse and complains of cool clammy skin, anxious, weak, hungry but nauseous, and slightly confused.

How do i perform a focused assessment on Mrs. Workman?

Focussed assessment on mr. workmen:-

01:- Obesity ( most of central area)

02:- Hypertension

03:- Eye haemorrhage exudates

04 skin acanthosis

05. Neurological activity  decrease may be absent light touch

06:- Feet dry , muscles atrophy

Note :- Cool clammy skin confusion anxious may leads to Hypoglycaemia in mr. workman so nurse assess the glucose levels (CBG).

Normal range of HB1AC is 3.4 % to 5% But mr workman's HB1AC is 10 % shows he is suffering from serious type2 diabetes .

Nurse observed lower extremities and assess:

Some wounds are formed in foot shows the mr workman have a diabetic foot .

Due to thier sedentary life style not maintaining personal hygiene . A sluggish wound form in foot area.
Nurse also observe level of glucose in body :-
There is increasing the chances of somgyi phenomena it actively when insulin is low and sugar is increased in body .

you take insulin before bed and wake up with high blood sugar shows the somgyi phenomena.

So whole above given is focused assessments on mr. workman.
