Discuss how nurses today communicate and collaborate with interdisciplinary teams and how this supports safer and more effective patient outcomes.
1) ans) In today’s health care system, delivery processes involve numerous interfaces and patient handoffs among multiple health care practitioners with varying levels of educational and occupational training
An interdisciplinary approach involves team members from different disciplines working collaboratively, with a common purpose, to set goals, make decisions and share resources and responsibilities.
In contrast, a multidisciplinary approach involves team members working independently to create discipline-specific care plans that are implemented simultaneously, but without explicit regard to their interaction
In all the hospitals now , there is a team for all the different systems and units which comprises of various different disciplines other than doctors and nurses. All these people come together to give proper care to the patient and to improve their life.
It is a very helpful method to put together the two methods which is interdisciplinary team and communication together inorder to provide the quality and good outcome of the hospice and palliative care.
This methods will help in the providing of the patient with proper care which evber required on the right time like if the drugs are required to be changed immediately the pharmacist will change it at once or if the lab records are to be checked immediately it will be done but the lab incharge on the very moment etc.
This will make the people happy to be getting all the required attention on and at the time immediately and will make th feel happy to be attended by all the different staffs of the hospital at once
Also the communication is a major and the basic part of the
treatment or care to be provided by the health care provider.
A proper communication with the people of the hospice care or the
palliative care will help them to gain confidence and to rely on
the healthcare person.
It will help them to share their feeling with the health care
people which will make it easier for them to treat and to
understand the problems faced by their patients.
This process focuses on understanding the reasons for failure and how to avoid it in the future. Most commonly, the process results in the development of safety guidelines, procedures, and rules for performance
Multidisciplinary teams convey many benefits to both the patients and the health professionals working on the team.
These include improved health outcomes and enhanced satisfaction for clients, and the more efficient use of resources and enhanced job satisfaction for team members
Older people in hospitals often have a number of different diagnoses and consequently have multiple and complex needs. Compared to younger age groups, a greater proportion of older people require an interdisciplinary approach to their care in order to deal with complex multimorbidity, social and psychological issues.
The best possible outcomes for older people are achieved through a consultative, collaborative approach to care that actively involves the patient, their family/carers and an interdisciplinary team.
An interdisciplinary approach can help avoid risk averse thinking by weighing up the risk against benefits for the patient.
An interdisciplinary approach can improve patient outcomes, healthcare processes and levels of satisfaction. It can also reduce length of stay and avoid duplication of assessments, leading to more comprehensive and holistic records of care.
The opportunity for discussion created by interdisciplinary care planning can be used for the patient, their family and carers to develop their ongoing plan.
