1. How can nurses collaborate with the inter professional team members to protect and provide care for the patient with under nutrition or obesity.

Under nutrition can lead to diseases like kwashiorkor, marasmus etc. Nurse plays an important role in patient care. She is the person who watches every step of the patient and provide therapy given by the physician. If a nurse observe any nutritional deficiencies or obesity conditions in patient while providing treatment then she immediately consults and provide information to the respected physician. If not possible then she share her views to the physician during medical ward rounds. She write all the medical and medication history of the patient and presents the case in general nursing meetings or in monthly therapeutic meeting.

If the patients requests for a transition management. Then before finalizing nutrition therapy of the patient identify all the needs of the patient like financial efforts, time, relationships, psychological behaviour, age, gender and other chronic health conditions. After reviewing all these then develop a care plan regarding his health benefits.

Nurses can teach adults by promoting campaigns in the schools, college's and in public places. They can add a slogan like healthy food give healthy life. They can say about what type of food adults should prefer whay type of food they should not prefee. Making a chart and say about nutrition food and colories of each food. Daily food intake, Physical activities.
They can also say adults about nutritional disorders. How undernutrition can effect our healthy life style.

The nurses can interpret adults by asking about the history of medical and medication conditions of adults. They will check the electolytes in the body. They can interpret by knowing about the patients life style and dietary habits.

After reviewing the case and after providing the therapy to the patient. The nurses will write indent on the end of the case sheet. If they observe any interventions they write down the interventions and submit them to the nursing headoffice and to the respected physician.
In a case of nutrionql deficiencies the nurses will take utmost care and provide better treatment to the patient. The nutritional deficiencies are generally seen in the patients with 1-5 years of age. The interventions observe in these type of patients are electrolyte disturbances etc. If they observe any interventions then immediately provide alternative treatment to the patient by consulting the physician.

The nurses will prioritise the patient care in nutritional deficiencies based on their health condition and age group.in most of the hospitals they maintain seperate ward for the nutritional deficiency disorders. They check the meal and provide information about daily intake of food. Some of the nurses may provide week wise chart about dietary supplements to the patients.

While providing total parentral nutrition to the patient: 1. Oserve and record sodium, potassium, chloride and electrolyte levels in the body.2. observe the patient dietary habits and life style.3. have look on edema and polypharmacy.

The general procedures involved in bariatic surgeries are sleeve gastrectomy, gastric bypass etc. These patients generally required hospital stay after the surgery. The interventions observed by nurses in thses patients are vitamin deficiency, protein deficiency and mineral deficiency.
