It is important to share information with the health care team about the dying person's preferences and goals of care. Reflect on what you would want your family and the health care team to know about you, if they needed to care for you when you were dying. For example, location, interventions, people present, level of medication and so on
The role of a nurse during the active dying phase is to support patient and family by educating them on what they might expect to happen during this time , addressing to their questions and concerns honestly,being an active listener and provide psychological ,emotional ,spiritual care and guidance.
