Fundamental planning strategies :-

1.In this apps every patient's have a I'd no. On this patient's having all data regarding demographic data,medical history,medication,allergic history,immunization status,vital signs ,laboratory test results,radiology test reports etc.

For that risk of data replication is avoided,decreased the risk of lost paperwork,easily analyzes patients condition.

2.In this app there is a table for medication timing & dose,that is updated during patients discharge.& there is a alarm setting during the time of taking medicines. For that patient can't forget to taking medicine.

3.one column is for diet chart of the patient.Whole diet chart about what types of food in which amount patient is taken is mentioned.

4.one column for Regular activity.For while patient is engaging with which type of physical activity must be updated regularly.

5.Daily weight,pulse,Resp rate,Blood pressure is measured & recorded in this app by patient or patient family.

6.If any problem occur,any physical disturbances occur then instant call doctor & take online consultation.After analyzing all data (that are recorded in home daily) if doctor thinks patient need to admit then he admitted in the hospital.

7.Follow up date is mentioned in app & patient is taking online appointment for follow up.

8.Health education is given & alarming signs are tell them for prevent any mishap by day to day through app.

9.If patient having any quiry they can ask in a "Question-Answer section",which is answered by any health care giver.

Thus we can prevent the readmission of chronic heart failure patient ,increase patient satisfaction level *improves outcomes.
