Patient got a skin tear during surgery, what teaching do you give for discharge?transparent dressing or none adherence dressing?

SKIN TEAR---- They are traumatic in nature caused by mechanical forces during any procedure or removing any adhesives.
Nursing intervention given to patient during discharge are------
1. Evaluate the skin around the wound , check the changes in color, swelling, warmth,etc.
2. Proper care of tissue is needed.
3. For tissue care always use sterile dressing , wet it with saline before removing it.
4. There should be no unnecessary skin exposure in wound site.
5. Take proper medications (antibiotics) as prescribed by Doctor.
6. Avoid rubbing the area near wound.
7. Proper nutritional diet , hydration , hygiene maintenance is instructed to patient to maintain skin integrity.
Dressing is important for fast wound healing , protection from injury, for patient comfort, it control moisture, prevent secondary infections.
Type of dressing given depends on site of trauma, extent of injury, duration of healing.like transparent dressing is used for skin tear which are superficial, not use in infected wounds. They are impermeable to bacteria or liquid and permeable to gases. They are adhesive dressing.
Non adherence dressing are used for skin which are sensitive in nature and are wound whose healing is fast. They cause less trauma on removing from injury site.
I think transparent dressing is better option for skin tear , but now a days hydrogels, hydrofibre are used because they causes less pain.
