Pyelonephritis is a bacterial infection of the renal pelvis,tubules and interstitial tissue of one or both kidneys.An incompetent ureterovesical valve or obstruction occuring in the urinary tract increases the susceptibility of the kidneys to infection because static urine provides a good medium for bacteral growth.Bladder tumors,strictures and urinary stones are some causes of obstruction that can lead to infection.Since the patient is hospitalized fluid intake and output chart are carefully measured and recorded.Patient is encourged to drink 3-4 litres of fluids per day to dilute the urine,decrease burning on urination and prevent dehydration.The nurse assess the patients temperature every 4 hours and administers antipyretic and antibiotics agents as prescribed.patient teaching is focused on prevention of further infection by consuming adequate fluids ,emptying the bladder frequently ,and performing recommended perineal hygiene.The importance of taking antimicrobial medications are stressed as is the need for keeping follow-up appointments.
Untreated urethritis does not resolve in its own ,resulting in blockage due to build up of scar tissue in the urethra.The surgical treatment for urinary stricture is urethral dilatation,internal urethrotomy,urethral stents.Patients are placed on bedrest following surgery under observation for 24 hours.Antibiotics are administered for 24 hours .Inorder to prevent bladder spasms antimuscarine agebts are given.Wounds should be washed with soap and water daily after drains are removed.
