Mr. Edwards is 20-year-old male patient who is admitted for treatment of recurring pyelonephritis (kidney infection) and surgical treatment of a urinary stricture, which has decreased the urinary stream. Mr. Edwards has paraplegia; he is paralyzed from the waist down secondary to an automobile accident when he was 16. He came by ambulance to the hospital, leaving his wheelchair and wheelchair pressure-relieving cushion at home. According to the nursing history, the patient is a nonsmoker and he does not drink alcohol or take any illegal drugs. What nursing considerations should be made for Mr. Edwards related to his disability?
Ans:
Pyelonephritis or kidney infection is mainly caused due to his paraplegia and bacterial growth due to his immobility.
Nursing considerations:
1. Firstly give him emotional and psychologucal support. And encourage him to be always happy and do his hobbies with hands.
Tell him that his hands are fully active, try to massage his lower body parts will improve blood circulation. Thus reduces the infection formation or reduce the bacterial growth.ROM exercises or passive exercises are best to improve the flexibility of bones and muscles too.It also helps to improve the mental strength also.
2. Similar important consideration is to drink lots of water, to compromise his fluid loss.Dehydration helps bacterial growth.So always stay hydrated.
3.Another main important consideration is timely medications (antibiotics ).Medicines should be taken at time.
4.Another nursing consideration include preventing bedsore while in hospital.Frequent position changing and assessing skin for its intactness.
5. Assist him while going to toilet and each and every daily activities as his wheelchair is left at home.Always take care while transferring him to the hospital wheel chair.Prevent falling while assisting. Remember to lock the wheelchair before transferring.Check whether the footrest is in position to prevent falling by tripping on them.
6.Always clear the clutter on the walking way to avoid slipping.Bathroom tile also the floors also should be slipping free.Take care of that before transferring.
7.Maintain good gait and position to avoid strain.
