Ans: Error-It is defined as the something done which is not correct or accurate.

Medical error-it is error happened in the medical which is preventable adverse effect of care.

The four errors are described below:-

COMMISSION ERROR- This error occured when doing something wrong in medical which can be affect to the patient.

Example-doing surgery in wrong site. This is one of the example of commision error.

SATISFACTION SEARCH ERROR- it is the error which can occur mostly in radiology department. It is the premature closure.

Example- failing of radiologist to search find more abnormalities while the initail one is found. So here the radiologist are failed to find more abnormalitues and close and takeout the result and gives error.

VERTICAL LINE ERROR- this error occured from routine, repetitive process which emphasize economy and efficacy.

Example- doing of diabetes creening to all the pregnant women.

"LAST BAD EXPERIENCE" ERROR- error cause patient death or injury.

Example- transfusion error
