1. Recall Ken Washington from the beginning of Chapter 35. Now that you have completed the chapter, answer the following questions regarding his case.
1. Is it significant that Ken had a urinary catheter in place for 6 days while he was in the hospital?
2. Dr. Buckwalter plans to send Ken home with a urinary catheter in place. What information can you give him to help him prevent infection?
3. You note on the chart that Dr. Buckwalter wants to see Ken again in 2 days. You ask Ken to schedule an appointment for that time. Ken’s wife states that they are going out of town for a week and will not be able to return until after that time. What should you tell Ken about making an appointment for more than a week?
ANSWER:-
(1) Case scenario is not given ,so not able to answer the significance of catheterization.
* If the patient had any disorders of urinary system or problems affecting urinary elimination it is significant.
(2) Information to prevent infections :
* Urinary catheter is always a source of infection if not taken care appropriately.
* Infection can easily enter the urinary tract through an indwelling catheter,so care of urinary catheter is very important.
* Advice the patient to keep the catheter clean and dry by gently washing with warm water and soap during daily shower.
* Wash the perineal area with soap and water atleast twice a day and dry well.
* Always keep the tubings and collection bag below the hip level.
* Instruct to avoid lifting the catheter tubings and collection bag above hip level ,it will results in back flow of urine and increases the chance of infection.
* Anchor the catheter to inner thigh to prevent it from moving in the urethra.
* Never disconnect the tubing , - the closed system helps to prevent entry of infection.
* The drainage bag must never touch the floor ,it should be always kept up from floor level.
* Ensure tube is not kinked or twisted and urine is flowing freely.
* Empty the bag through the drainage spout when ever it fills 1/2 and close the spout after emptying.avoid contamination of the drainage spout.
* Wash hands before and after handling catheter tubing or drainage bag.
* Encourage fluid intake and Keep a watch of fluid intake and urine output.
* Observe the amount and color of urine to identify any changes as early as possible.
* Report immediately if any fever,lower abdominal pain,burning sensation,cloudy urine,less or no urine drained etc.
(3) An assessment is needed within 2 days that is why doctor asked for follow up. This is to evaluate his progress in recovery , response to treatment and to identify any other problems like infection.So it cannot be after along time like after one week.It is alway safe to have the review within short period .once it is confirmed by the doctor that he is improving ,then you can plan your outing..
