Now that you have completed a series of assignments that have led you into the active project planning and development stage for your project topic on diabetic ulcer,briefly describe your proposed solution to address the problem, issue, suggestion, initiative, or educational need and how it has changed since you first envisioned it. What led to your current perspective and direction?
1 Foot ulcers are one of the major complication of poorly controlled diabetes resulting from tissue breakdown due to poorly controlled diabetes and is commonly found under the big toes.The first thing about diabetic ulcer is drainage from the foot which we often fail to notice the initial symptoms.The skin should be assesed for dryness or breaks.The feet are cleaned with warm water and soap.Excessive soaking of the feet is avoided .The feet are are dried thoroughly,especially between the toes and lotion is applied to the entire foot except between the toes.For patients who are confined to bed ,the heels are elevated off the bed with a pillow placed under the lower legs and heels resting over the edge of the pillow.A bed cradle may be used to keep the bedcovers off the feet of the patient with diabetic neuropathy.Dermal ulcers are treated as indicated and prescribed.The nurse should promote optimal blood glucose contro in the patient with skin breakdown. Signs of a diabetic ulcer is not obvious.it happens that ,most times it wont show any symptoms of ulcer until the ulcer has become infected.if there is any discolouration it needs to be noted especially if the tissue has turned black or if there is any pain around the surrounding tissues.
it has now become more easier to identify the seriousness of your ulcer on a scale of 0 to 3 using the following criteria. 0 -no ulcer but foot at risk ,1-ulcer present but no infection ,2-ulcer deep ,exposing joints and tendons,3-extensive ulcers or abscesses from infection.The most common cause of diabetic ulcer is due to poor circulation.The healing process of an infected foot ulcer will be delayed if the bood glucose level is rising.Nerve damage decreases the sensitivity to foot pain resulting in wounds with no pains and hence can cause cancer.But we can protect a diabetic patients foot by asking him to wear diabetic shoes,casts,food braces,compression wraps and shoes to prevent corns and calluses.Debridement is another treatment option.Footbaths,disinfecting the skin around an ulcer,keeping the ulcer dry,enzyme treatments ,dressings containing calcium alginates to inhibit bacterial growth .
