I have worked in hospital ambulance nurse and hone care so I think I'm the right person to answer this question since I have experience with all section
1- hospital
In hospital there are two reason like acute cases and chronic cases
In chronic cases doctors and care givers will get enough time to explain the patient condition and relatives can mentally prepare for bad outcome.
In acute cases it's hard to make comfortable their relatives especially if the victim is young.sometimes it's need any help from social worker or psychologist to help the relatives to convince them.
2-palliative service
Either it's home based or facility in this cases patient condition is well aware and they are prepared about any bad outcome.in this case care givers wont need to take any extra effort to convince the family about death in majority cases
3-disater or any other immediate death
This is the most challenging for care givers it start with identification of victim .And it's really not easy to break this aad news to relatives, usually athey won't give clear information by phone since it can cause any mental stress.so they try to convince them to come hospital so so is admitted here.once they reach there they will adress this to family especially if they are old .
