Antonia is a client in the psychiatric unit with a
diagnosis of paranoid schizophrenia. She has a history of
noncompliance with medications. Today she is very delusional and
refuses her breakfast. She approaches the nurse and states, "God
told me to warn you that meteors are descending toward the earth.
They will get into the brains of all women to prevent the
overpopulation of the outer planets". Antonia places frantically
and finally crawls under the coffee table in the day room holding
her arms over her head.

How can the nurse best approach Antonia regarding her behavior?
Common symptoms of paranoid schizophrenia include auditory hallucination[hearing voices or sound effects] and paranoid delusions [believing everyone is out to cause the sufferer harm]
In paranoid schizophrenia delusions of persecution and delusions of being followed found to be the most common delusional themes in the paranoid schizophrenia.In persecutory delusions the individual think that harm is occuring ,or going to occur.the individual also thinks that the perceived persecutor hsa the intention to cause harm
Here the patient is experiencing persecutory delusion she thinks that something harm is occuring- ''God told me to warn you that meteors are descending towards the earth.They will get into the brains of all women to prevent the overpopulation of the outer planets ''
HOW CAN APPROACH
-Validate any part of the delusion that is real- for example - may be God has plan to reduce overpopulation but i did not think that meteors are descending towards the earth
in initial assessment ask the patient to talk about the delusion and obtain details by asking the following question
Who is going to hurt
could you thimk why
how might it happen
do not maintain that what the person is thinking is wrong instead ,show that you respect his or her point of view regardless of whether you agree
listen quietly untill there is no further need to discuss the delusion
do not expect that rational thinking will have an effect on the person's delusions.If you debate the delusion ,the person may expand the details to counter your arugment
develop a symptom manage strategy
monitor recovery,compliance and medication and general physical health .educate the patient aboutpossible side effects of medication and work with the person to develop appropriate actions to address any issues
