1. Pneumonia Pt , has no history of DM , why is her serum glucose elveated?
2. Why is the same pt , have no fever?
3.Is there a sig probabilty that the patient got a UTI , that caused her pneumonia? She has urine incontinence and a clean urinealysis
1 As mentioned patient is suffering
from pneumonia with no history of DM still she shows signs of
elevated serum glucose level, it might be due to community acquired
pneumonia (CAP). and High risk case should be handled with CCU or
ICU admission. Due to CAP serum glucose may be elevated without any
DM.
2. CAP generally occurs due to some pathogens and rarely with viral
origin. If this is the case of viral originated CAP then they will
not rise the level of fever causing factors to that much extend and
will show no signs of fever.
3. As there is infection which could spread to kidney and kidney
function may gets hampered or may cause renal failure too. So an
untreated UTI may spread to the kidney, causing more pain and
illness. It can also cause sepsis. The term urosepsis is usually
used to describe such kind of sepsis caused by a UTI. Sepsis and
septic shock can result from an infection anywhere in the body,
such as pneumonia, influenza.
