The client’s X-ray shows consolidation in the left lobe, and physical assessment reveals diminished lung sounds. The nurse administers acetylcysteine as prescribed over nebulizationn via edotrachial tube. Which therapeutic response of this medication should the nurse expect?

    A. Hypotension

    B. Bronchodialation and wheezing

    C. Unpleasant smell when using the medication

    D. Increased sputum, requiring suctioning

Solutions

Expert Solution

D.Increased sputum required suctioning

Therapeutic effect

  • Acetylcysteine has mucolytic action that  liquefies or dissolves mucus.
  • Acetylcysteine has free sulfhydryl group, which reduces the disulfide bonds in the mucus matrix and lowers mucus viscosity. Acetylcysteine liquifies pulmonary secretions, and the increased volume produced can occlude the airway. The mucus may have to be removed by suction if the patient is unable to adequately clear the secretions. Here the patient is intubated and need suctioning.(Avoid prolonged suctioning also)

Why other choices are not valid as right answer.

All other choices are side effects or adverse reactions not therapeutic.

A. Hypotension

Hypotension is a side effect of the drug not therapeutic effect.

B. Bronchodialation and wheezing

This also a side effect in this case not a therapeutic effect.

C. Unpleasant smell when using the medication

The drug have an unpleasant smell likely from the sulfur present in the drug.
