Assessment :-

Subjective data:- Patient will have complaints of painless sore on the genitals,rectum or mouth.They also might have multiple sexual partners for the past 3 months.They will be complaining that they are afraid of having STD.

Objective data :- The patient will have painless ,red, ulcerated sore on genitals, rectum or mouth.The blood sample will be positive for syphilis.

Nursing diagnosis:-Risk for infection transmission related to contact transmission exposure, multiple sexual partners, evidence by positive blood for syphilis.

Goal:-The patient will be able to understand the disease transmission, management and prevention.

Nursing interventions:-

* Educate the patient to abstain from sexual activity to prevent contact transmission until the healing of the sore.

* Explain about the importance of prevention of sexually transmitted disease.

*Advise the patient to inform the sexual partners he/she had contact last 3 months and test for syphilis.

* Explain about the disease, signs and symptoms, management and its complication.

Evaluation:- Patient will verbalize that they understood the disease, management and prevention.

Other nursing diagnosis for syphilis:-

*Anxiety related to the disease process and its transmission.

Alteration in comfort pain related to sore on genitals related to disease process.

Potential for infection related to disease process.
