Debra is a 40-year-old woman who had gained over 50 pounds within the last 10 years. Tired of being overweight and being told she had no willpower, was weak, or that she needed to change her lifestyle – eat less and exercise more, she sought out medical treatment to help her lose weight and thus prevent health risks that could cause serious illness or even death.

The physician explained to her that he would not recommend starting weight-loss medications until she followed a diet subsequent to seeing a nutritionist. He said that she should follow-up with his office in 3 months for a re-assessment of her lifestyle changes (eg, healthy eating, and regular physical activity). Upon follow-up 3 months later, she had lost about 2 ponds.

Debra was referred to the Community Health Nurse for follow up management.

Social environment is the surrounding which we live in , such as, the home and neighborhood, the academy, the work place. It is confinement area of our body completely and mind majorly. A positive vibe out if this social environment make us feel alive and energetic. But at the same time, any sort of discouraging behavior from the regularly mingling people in the social environment will make us down, which will reflect in the health status negatively. Whereas physical environment includes the natural occurrences like the water,river, forest air, etc. Fresh and clean water and air are mandatory for a healthy and well-being. Pollution free atmosphere secures the health from infections too.

Regarding health promoting behaviors, loss of determination , dedication and work eventually reduce the consistency in it.

STAGESOF CHANGE MODEL FOR DEBRA

  • Precontemplation:- here she is not aware of the positive outcomes of the healthy diet and gradual weghtloss. Here we describe the anticipated possible health related issues like cardiovascular disorders, dyslipudemia, even death, being obese.
  • Contemplation:- this is the stage where Debra comes to be aware of her health status and the possible danger.
  • Preparation:- This is where Debra prepares her mind and body to accept the reality of her health and work for that accordingly. Small sessions of walking is allowed here.
  • Action : the actual practice come here. Debra is supposed to be very dedicated and follow the diet and exercise now.
  • Maintenance: this is the point where Debra is not supposed to even think of her old lifestyle and maintain the healthy routine.

Paralysis and subsequent maggit infested bed sores was a clinical situation I came across while being an intern, which obstructed the patient from leading a healthy life style.

B) Certain health care and fitness apps are available in the internet which helps the Patients for monitoring and intervening the status of healthy routines, diet etc.
