D.V. is a 34-year-old woman who had a ruptured appendix 10 days ago with subsequent peritonitis. She
was discharged to home care yesterday, on postoperative day 9, with a left peripherally inserted central
catheter (PICC) for IV antibiotic therapy. You work for the home care department of the hospital. You have
been assigned to D.V.'s case, and this is your first home visit. You are to do a full assessment on D.V. During
the assessment you notice a large ecchymotic area over the right upper arm. You ask her whether she fell
and hit her arm. She tells you, "The nurses took my blood pressure so many times it bruised."

  • Now she is presenting with a large ecchymotic area over right upper arm."

ASSESSMENT

My assessment of the situation is that the

"patient D. V might have developed thrombocytopenia and and may land into disseminated Intravadcular Coagulation due to Peritonitis induced septicaemia or antibiotic associated thrombocytopenia."

RECOMMENDATIONS

I recommend that " an urgent laboratory test for General blood picture, blood coagulation profile, D dimer test, Liver function test kidney function test should be done. "
