4.a)Criteria for classification of bariatric client :

According to BMI general population is classified into 5 categories:

  • Underweight - BMI<18.5 Kg/m2
  • Normal weight - BMI -18.5 -24.9 Kg/m2
  • Class I obesity/ overweight -BMI : 25 - 29.9 Kg/m2
  • Class II obesity /obesity - BMI : 30 - 39.9 Kg /m2
  • Class III obesity / extreme obesity - BMI >40 Kg / m2

From the above classification the BMI criteria for bariatric client is :

  • BMI is over 33 and suffer from type II diabetes.
  • BMI is over 35 and have additional health disorders related to obesity(hypertension ,respiratory disorders , lipid abnormalities etc).
  • BMI is over 40.

Other criteria include :

  • Medical history.
  • Dietary habits ,life style and previous weight loss attempts.
  • Physical activity or inactivity.
  • Psychological disorders.
  • Personal commitment.

b).COMMON CAUSES OF OBESITY :

  • Over eating and physical inactivity.
  • Genetic cause - Leptin deficiency .
  • A diet high in simple carbohydrate - carbohydrates increase blood glucose levels which stimulates insulin release by pancreas and this insulin promotes growth of fat tissue and cause weight gain.
  • Frequency of eating - Overweight people eat leass often than people with normal weight.
  • Medications- Antidepressants ,anticonvulsants,corticosteroids ,oral contraceptives etc cause weight gain.
  • Psychological factors - Many people eat excessively in response to emotions such as sadness ,stress and anger.
  • Certain diseases such as hypothyroidism ,insulin resistance ,polycystic ovary syndrome contribute to obesity.

c)3 ways a CCA can build trusting relationship with obese patients:

  • Biomedical: Medical data gathering and medical education and counseling.
  • Psychological : Psychological data gathering and psychological education and counselling . Psychological counseling includes -a)Communication : Good communication behaviours such as empathy encouragement ,psychological talk improve patient satisfaction and adherent. b)Positive attitude: Physicians attitude towards the patient provides information and expresses more positive effect.
  • Rapport building - a)Positive rapport building : Can be acheived by humour ,complements ,approval and agreement .b)Emotional rapport building : Empathy, concern, reassurance ,partnership and self disclosure.c)Social rapport building : Personal remarks ,non medical social talk.
