Your patient is a 23-year-old female with a history of headaches. This morning she was experiencing a really bad headache as her boyfriend was driving her to the university. He thought she was sleeping and by the time they got to the school, 45 minutes later, she was not waking up. She was diagnosed with a cerebral aneurysm and you are taking care of her in the Neurological ICU.
The family is approached about organ donation. The patient indicated on her driver's license that she wanted to be an organ donor. Her mother refuses to consent for organ donation.
Brain dead evaluation is basically clinical evaluation combined with neurological evaluation. If clinical evaluations like gag reflex fails, body temperature falls, blood pressure falls, respiration is shallow or assisted,
Neurological: - absence of brain stem reflexes like Absence of pupillary response, ocular movements, corneal reflex, no response of facial muscles, and last absence of respiratory drive
The procedure is called as brain-dead certification, 2 certificates 6 hours apart. This includes identification of history or physical examination findings that provide a clear etiologic of brain dysfunction.
Neurologists, neurosurgeons, intensive care specialists and clinicians with special certificate about organ donation can approach.

If daughter has registered a decision to donate, there is no legal right for mother or family to override her consent; however, families are still involved in discussions about organ donation. Convinced to fulfil the last wish of the patient.
a law known as "First Person Consent" in this if the daughter is registerd as a donor then no one can override that decision.
There are no ethical constraints as the donor has first person consent.
