Description of the client/identifying. information
Mr. Vincent a 68-year-old, widowed Caucasian male, of Irish descent. He is retired and lives alone in a one-family home. He was referred to Senior Care to evaluate his current situation and determined what levels of services can be provided for him.
Reason for Referral
Mr. Vincent was referred to Senior Care for an evaluation to determined the level needs that can be provided to help him assist with his daily needs. Reports of recent falls, seldom attend his Doctor appointments and the grocery store. In addition, Mr. Vincent has been isolating himself.
Physical Function:
The client seems to be well-groomed. He wore black slacks and a tan sweater. No indication of limitations with his physical performance. Reports of recent falls in the last month no injuries accord. No medical concerns or substance abuse. He admits drinking two glasses of scotch a night.
Cognitive perceptual functioning:
Mr. Vincent graduated from high school then went into the Navy. He left the Navy because he served his four years. He was honorably discharged. After high school he joined the Navy. He left the Navy after four years. He was honorably discharged.
Mr. Vincent worked for Bath Iron Work. He was an ironworker and build ships for the building company in Bath Maine. He worked for the company for about 40 years and built ships.
Emotional functioning:
Mr. Vincent indicated he has been isolating from others. He exhibits signs of depression and much anxiety. There are no indications or reports of mania behavior, delusions or hallucinations.
Social & family functioning:
Mr. Vincent worked lots of overtime. His wife did not have to work. He was a very traditional male. He believed the wife should stay home while the men worked in support of the family.
There is a lot nonverbal communication between Mr. Vincent and his two children. Mr. Vincent has not seen his children or grandchildren since his wife died three years ago. He expresses much anger towards his children because they do not make an effort to call him or come by and visit. He seems to hold a grudge against because of this so he does not communicate with them either.
Behavioral functioning:
Mrs. Vincent reports feeling angry. He has indicated on occasions having multiple confrontations at the bar. His behavior is showing signs of impulsivity. Mr. Vincent expresses much negativity towards his family. No history of legal concerns.
Motivation:
Mr. Vincent agreed to a meeting with me and his children. Mr. Vincent reports some life setbacks. He is willing to address his issues willing to try to build a relationship with his children and grandchildren. I do feel Mr. Vincent is set in his own ways. He seemed extremely critical about making changes. It will not be easy for him even though he is willing to try.
Spiritual/religion:
Mr. Jones considers himself a Catholic. Since the passing of his wife, he has not attended church services. He does report he believes in God but does not practice his religion faithfully.
Environmental systems:
Mr. Vincent talks about how there is a woodshop downstream from his house and the hobby shop. His house is within walking distance to downtown.
Culture factors:
Mr. Vincent is an Irish Catholic.
Strengths:
Mr. Vincent Enjoys building boats and model airplanes. Being an ironworker, he has the ability to concentrate and imagine how things will look if it is moved around or parts are rearranging. To regain his passion, it will take his mind off of other stressful environments and concentrate on his skills, and will allow him to manage his downtime or (isolation) hopefully resulting in a more positive direction. Mr. Vincent was extremely passionate about his work and made sure his children were financially set and involved in helping them enrolled in extracurricular activities.
Barriers to services:
Mr. Vincent shows no expressions of emotions of feeling. He indicated to me, when his wife died, he did not even cry. Mr. Vincent is a difficult man to read so say I question if he is only being honest/. What prevents Mr. Vincent to get involved with the services provided by Senior Care is his desire to be independent. Ironworkers are considered to be rare individuals. They need to be physically and mentally in shape.
Assessment and Formulation:
Mr. Vincent is struggling with the loss of his wife and continues to still grieve. Not letting go of his past he instead isolates himself from his friends and family. I feel he is suppressing his emotions so deeply which transforms into anger. This has caused difficulties in his relationships with his children and has developed a negative relationship with his grandchildren. Until our next meeting or possibility a meeting with his grandchildren or children I will be able to assess more with input from the immediate family.

OBJECTIVES
Mr. Vincent must be assessed for both psychological and physiological needs, for further care provision includes improvement in better communication with his children and others,social interaction hence come out from his social isolation.moreover managing extreme anger in assisting him to manage his emotions.
ACTION PLAN
a) Nursing plan for impired social interaction
1.Engage the patient in activities which needs very less concentration.as the depression makes a person less concentrated on his activities
2.gradually make the patient to take part in activities which need minimal concentration like gross motor activities
3. finally involve the patient in group discussion and different kinds ok group therapies
4.advice his children to make him take part in self help group once reach home.
b) Nursing plan for grieving
patients detatchment from family members are the evidence of grief. therefoe;
1.discuss the loss with the patient to admit it and comeout of his denial stage
2.assess the cultural practice and religious practice as he was a kind of conservative parson and would go for religious practices earlier by which to understand the way he accept the grieving process.
3.assess and reffer the patient if any theraputic measures required.
c) Nursing plan for anger
1.Create a trustful relationship with the patient.
2.Identyfy the situations which triggers anger
3.Rduce environmental stimulation like extreme light and noise.
4.teach the patient the ways to gain self control over his anger.
d)provide advice to his family menbers to deal him to come out of his current situation.
