Case study 1
Janine works as palliative care nurse at a regional health facility and relates the following conundrum. Janine describes looking after Jack who recently returned to the palliative care service after a further unexpected exacerbation of his respiratory lung cancer. Jack is rapidly deteriorating with little tolerance for any activity and cannot undertake any activities of daily living. Shirley, his wife is constantly at his side offering as much support as possible. Jack is highly dependent and has several HCP ‘s attending to him including physiotherapists, doctors, nurses, occupational therapist and other allied health workers including kitchen and cleaning staff. Jack received care for the past 6 days at which time he was in and out of consciousness and his lucid moments were characterized by confusion due to his severe low oxygen saturation rates. Whilst confused he tried to leave or get out of bed and required constant supervision, and at night restraint. Shirley was at his bed side frequently. Shirley in conversation with Janine, complements her and the team on the care that Jack has received, and reminisced with her and over the last few weeks they had spent together on a cruise to New Zealand and surrounding Islands. Janine realize that cruise was associated with a severe outbreak of COVID19. Janine asks Shirley, “why didn’t you let us know of your trip”? Shirley states that if she did, she might not have been allowed to see Jack. Subsequently, Shirley was not allowed to visit Jack, Jack died 2 days later. His pathology result was positive for COVID 19. Janine has explained the issue to all the staff associated with the care of Jack. The paramedics who cared for of Jack were also informed of his infective status prior to his death.

Henry has a few thoughts and options to explore, and these include taking his long service leave for the next 8 weeks. Taking leave without pay for a period of six months. Henrys central concern is around protecting his children and wife from being infected with COVID 19 as the risks of workplace exposure are very real. Henry finds himself in conflict with his duty of care and worries about his family should he continue to work. He discusses his quandary with his line manager, who suggests that should he contract this illness he will be supported by the health facility and that he should secure accommodation away from his family whilst he works. Over the past week Henry has become even more distressed with having to work with the ICU team to decide how they ration the beds available to them and how they prioritise who receives care in their unit of 10 beds.

Staff is not organized what so ever!!! My mom sees the doctor for asthma shots once a month, shot takes 2 seconds to give. Yet she sits in the waiting room for over 2 hours!! She told the front desk staff how long she had been waiting and the response was… oh you still have not gotten your shot… This behavior is unacceptable!!!! They do not value their patients’ schedules nor health for that matter. Seeing as though my mother can’t go with out this shot!

So, bottom line is that mental health issues like excessive anxiety and depression are no joke – either at a personal level or at the macro level of the US healthcare system.
But while millions of people suffer from mental health issues, it is also noted above that millions of suffering people are left undiagnosed and untreated.
So what should we do? How can we significantly improve our ability to help people in need of mental health treatment?
What should we do about the stigma that some attach to seeing a mental health professional?
What about the equal stigma of being labeled as someone who is “mental?”
How do we, as healthcare professionals, best reach out to those that desperately need assistance –without making anyone think we are looking for “crazy” people?
Overall – what is the best way to address the large and underserved need for mental health care?
